STEP ONE: COMPLETE MANDATORY REGISTRATION FORMS

• Read SMHS and SMUSD Required Readings, and Titan Guide.
• Download the fill-able PDF file: MANDATORY REGISTRATION FORMS
• Type in your responses on this form.
• Print the document RIGHT AWAY – your responses will not be saved
• Hand sign every page where required.
• Staple your checks to their respective forms (Titan Package, PTSA Membership and Donation Form, San Marino Schools Foundation).
• Return the forms by mail or by dropping them off at the APO Office before August 13^th.
• Make directory changes at https://bit.ly/smhs2013
• For students with caregivers other than guardians or parents:  
  You are required to annually update your caregiver form at the District Office.  If you fail to do so prior to
  Registration Day (Aug. 20^th), your student WILL NOT BE ABLE TO RECEIVE HIS/HER SCHEDULE.

• Temporary Custody of Children
  - Retain form and complete ANYTIME your child is under the care of another while you are away from home.  
    This form should be turned into the Attendance Office prior to your leaving.
