In our ongoing efforts to make the First Day Packet procedure more
accurate and efficient, all of the mandatory forms required for
Software Requirements: You must have the most updated Adobe Acrobat Reader to complete the fill-able forms. Attention Apple users: PDF documents may open in “preview” and will not be fill-able. You will need to download the forms using Adobe Acrobat Reader. To download Adobe Acrobat Reader, click here.
- Read SMUSD Read-Only Documents.
- Download the fill-able PDF file titled “Mandatory Forms” below.
- Type in your responses on these forms.
- Print the document RIGHT AWAY – your responses will not be saved. If you have several children at the school, please modify the form accordingly and reprint for each child.
- Hand sign every page where required.
- Place your check payment along with the appropriate forms into the respective envelopes included in the First Day Packet (Carver PTA Order Form, Instructional Materials Donation, San Marino Schools Foundation Donation, Partnership for Awareness, etc.)
- Return these mandatory forms along with any others included in the First Day Packet in the manila envelope provided to your student’s classroom.
