Instructions for On-Line Critical Forms

• Download the "Critical Registration Forms" fill-able PDF file below.
  
• Type in your responses on these forms.
• Print the document RIGHT AWAY – your responses will not be saved.
• Hand sign every page where required.
• Staple your checks to their respective forms. (Titan Package, PTSA Membership and Donation Form, SMHS Foundation Annual Campaign)
• Return the forms along with the other forms from the First Day Packet by mail or by dropping them off at the APO Office before August 16

Software Requirements:  You must have the most updated Adobe Acrobat Reader software to complete the fill-able forms.  Attention Apple users:  PDF documents may open in “preview” and will not be fill-able.  You will need to download the forms using Adobe Acrobat Reader.  To download Adobe Acrobat Reader, click here.
