Mandatory
Forms for HMS Registration are now on-line
In
our ongoing efforts to make the registration procedure more accurate and
efficient, almost all of the required forms to register at Huntington Middle School
are now available ONLY on-line.
Software
Requirements:
You must have the
most updated Adobe Acrobat Reader to complete the fill-able forms. Attention Apple users: PDF
documents may open in “preview” and will
not be fill-able. You will need to download
the forms using Adobe Acrobat Reader. To download Adobe Acrobat Reader, click here. - Download the
fill-able
PDF file titled “Mandatory Registration Forms” below.
- Type in your
responses
on these forms
- Print the
document RIGHT AWAY – your
responses will not be saved
- Hand sign
every page where required.
- Place your
checks/payment along with
the appropriate forms into the respective
envelopes included in the First Day Packet (PTA Order Form, SM Schools
Foundation Annual Campaign)
Return the on-line
mandatory forms along with the other forms from the First Day Packet by
dropping
them off in the First Day Packet manila envelope at the HMS Front Office
or
returning them in the envelope to your student’s homeroom on the first
day of
school. The First Day Packet manila
envelope is available for pick up at the HMS Front Office starting July
12 and
includes many other important fliers and communications.
