2nd thru 5th Grade Registration Information

The next round-up registration date for the 2010-2011 school year for all grades will take place on Thursday, July 22, 2010, from 9:00 am to 12 noon in the District Office Boardroom. There will be another round-up registration date on Thursday, August 5, 2010 from 1:00 to 5:00 pm, also in the District Office Boardroom.   

All registration materials must be completed before registration.  The free Adobe Reader is required to view the PDF files.

STEP 1:  Important Information You Must Read First.  Please be sure to click on each section and read the information

STEP 2:  Print, Complete and Bring the Following Forms.

[A]  School Registration Form
[B]  TB Test Requirements & Test Report
[C]  Food Services Payment Form (optional)
[D]  Application for Free and Reduced-Price Meals or Free Milk

STEP 3:  Gather and Bring the Following Documents. Please be sure you have both the original and a copy of the following documents. (Chinese Version)

  1. CURRENT PROPERTY TAX BILL OR CURRENT LEASE AGREEMENT OR CLOSING ESCROW STATEMENT;
    AND

    4 CURRENT UTILITY BILLS (electric, gas, water, trash, cable, telephone [not cell phone])
     
    are required for proof of residency in the San Marino Unified School District.
     

  1. BIRTH CERTIFICATE AND/OR PASSPORT
    for certification of birth date.
     

  1. SOCIAL SECURITY NUMBER
    if applicable.
     

  1. COMPLETE, UP-TO-DATE, IMMUNIZATION RECORD
    signed by a physician. All the immunizations required by the State of California are listed in the registration packet. Our district strictly enforces the “NO SHOT - NO SCHOOL” policy.
     

  1. MANTOUX TUBERCULOSIS SKIN TEST
    for all students entering school in California for the FIRST time. This test must be given and verified in writing by a physician. The form is supplied for this requirement.
